Rename UNW_IA64_AR_25 to UNW_IA64_AR_CSD, as per SDM v2.1.

(_Uia64_search_unwind_table): Declare.
(_Uia64_find_dyn_list): Declare as "extern".
(_Uia64_get_kernel_table): Declare.
